Patient Privacy Rights

As outlined in the Notice, patients have
specific privacy rights, such as the right to
request a review of their health information
either during their hospitalization and/or
after discharge.

Hospitalized patients may contact the Patient
and Guest Services Department to request a
records review. Discharged patients may
contact Health Information Management to
request copies of their records.
The Notice describes each one of these
patient privacy rights in detail. These
rights include:
• The right to inspect and copy health
information that may be used to make
decisions about their care or payment for care
• The right to request an amendment of health
information
• The right to an accounting of disclosures

• The right to request restrictions
• The right to request confidential
communications
• The right to a paper copy of the Notice.

• The right to choose someone to act on their
behalf
