Reporting HIPAA Privacy Incidents

Workforce members are responsible for reporting immediately any
HIPAA Privacy Incidents through one of the following channels:
• Reporting the incident directly to your supervisor or leader

• Completing an incident report through the online Risk
Management Reporting tool

• Contacting the HIPAA Hotline: 786-596-8850 or
Privacy@BaptistHealth.net

• Reporting the incident directly to your facility’s Risk
Management or Patient & Guest Services or Human Resources
departments
• Calling the Corporate Compliance Hotline 1-888-492-9329 or
reporting the incident using the online Corporate Compliance
system via MyComplianceReport.com (enter access ID BHS)
