2020 Annual Required Education
To report a cardiac death:
• The family is informed by the attending physician regarding patient’s
death.
• The death is reported within one hour by an individual familiar with
the patient (bedside nurse, nursing supervisor, or social worker). The
patient’s chart must be available when calling to report.
• The Donor Referral Service Operator will request the patient’s name,
date of birth, race, admitting diagnosis, date and time of admission,
patient’s height and weight, past medical history, fluid/blood, WBC, and
social security number for identification purposes, to access the State of
Florida Donor Registry, and to assist in locating next of kin if necessary.
• To determine the patient’s eligibility as a potential donor, further
information required will include past medical and social history,
cumulative I/O’s, lab values, x-rays, medications and drug history. It is
permissible to fax such documents to the Tissue/Eye Coordinator for
further evaluation of patient.
• The Donor Referral Service operator will inform you regarding the
patient’s eligibility for donation.
• If patient meets final suitability, contact information will be requested
by the Tissue/Eye Coordinator who is responsible for contacting patient’s
decision-maker at a later time, to request donation.
