Suicide Patient Safety
Three-Step Suicide Risk
Guidance for the Urgent Care
Step 1: Identify
The staff identifies a patient at risk for
intentional harm to self or others.
Step 2: Observe
Provide 1:1 monitoring of at-risk patients
with continuous visual observation.
Step 3: Remove
Remove all environmental risk items.
Patient access to certain areas may need
to be restricted to prevent patients from
reaching items that could be harmful.
Environmental assessment: the staff need to be
aware of the environment of care and note areas
that could be dangerous for patients at high risk
for suicide. The patient will need a 1:1
monitoring and environmental items in the room
will need to be removed.
