Protected Health Information (PHI)




        Protected Health Information (PHI) is information that relates to the individual’s
        past, present, or future physical or mental health or condition; to the provision of
        health care to an individual; or to past, present, or future payment for the
        provision of health care to the individual; and either identifies the individual, or
        for which there is a reasonable basis to believe it can be used to identify the
        individual; and exists in verbal, written, and electronic formats.

        Baptist Health’s employees may only
        access, use, and disclose PHI for health
        care functions related to their job that
        include  treatment, payment, and/or
        health care operations (TPO).


        Employees must only access, use, or
        disclose information if it is required to
        do their job.
